2004 Goals For Ending Chronic Homelessness:

• Develop strategies to better integrate the Contiuum's service delivery plan with the State's mainstream services.
• Survey local agencies to determine the impact of institutional discharge on requests for service.
• Prepare and present a presentation on "What Works" by collecting data and identifying promising programs.
• Revise behavioral contracts to include community focused participation.
• Facilitate recognition celebrations to motivate and encourage clients who are successful at moving through the system.
• Link individuals and families who move to permanent housing with others in their new neighborhoods.
• Continue wrap-around training for Front Line Workers.
